Section B:  To be completed by the trainer. Section C:  To be completed if the employer intends to claim CITB grant – if so, the delegate will need to ensure that the seven-digit CITB Levy/Grant registration number, employer’s name, address and postcode are completed.  If this section is not completed, it will be assumed that a claim for the CITB Levy/Grant is not required.   We trust that you find all the above in order.
